# Declaration of Health

1. If the vessel has a valid certificate on board, tick the respective checkbox and enter the location and date it was assigned, then indicate if a re-inspection is required.
2. Check the tick box available to indicate if a vessel visited an affected area identified by the World Health Organization.
3. The follow sections are to be completed where applicable:
